This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. This E log book also reflects my patient-centred online learning portfolio and your valuable inputs on the comment box is welcome."I've been given this case to solve in an attempt to understand the topic of "patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with a diagnosis and treatment plan.
CHIEF COMPLAINT:
26 yr old, male mechanic by occupation came with c/o fever since 5 days.
HISTORY OF PRESENT ILLNESS:
The Patient was apparently asymptomatic 6 days back, then he developed fever
which is of high grade, intermittent type associated with chills relieved with medication .
No h/o vomitings, loose stools, burning micturition, abdominal pain.
No h/o of sob, cough, cold.
No h/o bleeding manifestations.
PAST HISTORY:
N/k/c/o of DM, HTN, CAD , TB, BA, Epilepsy.
No previous surgeries .
PERSONAL HISTORY:
Diet: Mixed
Sleep: Adequate
Appetite: Normal
Bowel and bladder movements are normal
Drinks alcohol occasionally.
FAMILY HISTORY:
Not significant
DRUG HISTORY:
Not allergic to any known drug

He is conscious, coherent and co-operative.
